How do I make an appointment and reach other service ?

We offer convenient hours of operation to ensure you have choices to meet your busy schedule. Simply call 214-426-3645 and follow the quick reference guide below:
Press: 0 for Family Practice, Adult Medicine, Pediatrics
3 for OB/GYN
4 for Pharmacy
5 for Medical Records
6 for Fax number or General Address
9 for Dental

What can I expect when I visit the clinic?

You can expect to be greeted with a smile, and provided the utmost attention by our customer service associates.  If you are coming for a visit with the doctor or dentist, you will be taken care of by the provider's nurse first;  You may receive some lab work depending on the reason for the visit.  You may also have your prescriptions filled at our on-site pharmacy.  And most importantly, our staff will be attentive to your needs and help you make the best decision about your care.

New patient forms to complete.

See instructions: Simply print off the New Patient Forms and compete the forms in advance.  Bring all of the forms with you along with your valid identification card, and medical or dental insurance card.  Completing these forms in advance should help decrease the time required to register you into our computer system. Select from Adult Patient Registration for or Child Patient Registration Form.  Both forms are available in English and Spanish.
